VA Rating Code Sheet

How to Obtain your VA Rating Code Sheet

A VA Rating Code Sheet (sometimes called a “VA code sheet”) is an internal document that shows the diagnostic codes for your VA rating as well as your conditions, percentages, effective dates, static or dynamic, et al.

Veterans have reported document requests taking up to two weeks to process before either being sent through the mail or faxed.

The following are methods you can use to obtain your document:

Ask Your VSO or Visit A VA Regional Office

The VA Regional Office and organizations such as the VFW, American Legion, DAV, and county VSOs are able to print out your Code Sheet by request during an appointment.

This is the fastest method of retrieval.

Call the Department of Veterans Affairs

Call the VA benefits line:

  • 800-827-1000

Ask specifically for:

“My VA Rating Code Sheet”
or
“My disability code sheet”

These documents can be mailed or faxed to you but the processing after submitting a request can take up to 2 weeks on average.

Submit a Privacy Act / FOIA Request

You can formally request your C-File (claims file), which includes the code sheet.
